Phacoemulsification in treating senile chronic angle-closure glaucoma

OBJECTIVE: To study phacoemulsification-caused changes of IOP and depth of anterior chamber in senile primary angle closure glaucoma (SPACG). METHODS: Phacoemulsifiation was performed on 130 eyes. 78 eyes of senile cataract without glaucoma history and signs were assigned to group A. Complicated cataract with SPACG (15 eyes) and normalized intra-ocular pressure (IOP) after filtration procedure comprised Group B. SPACG (22 eyes) received only phacoemulsification in group C and SPACG (15 eyes) phacoemulsification combined procedures in group D. IOP, chamber depth and number of medications were recorded pre- and post-operatively. RESULTS: In all four groups, chamber depth was increased and IOP was reduced after phacoemulsification. Mean of IOP in A, B, C, and D groups (pre/post operation) were (15.4 ± 3.7/12.5 ± 3.0) mmHg, (15.2 ± 2.4/10.7 ± 1.9) mmHg, (21.2 ± 4.5/13.4 ± 3.0) mmHg and (27.0 ± 5.8/14.8 ± 7.8) mmHg (all P < 0.01) respectively. Mean chamber depth in all groups before and after surgery were (2.75 ± 0.60/3.58 ± 0.27) mm (P < 0.01), (1.84 ± 0.51/3.48 ± 0.37) mm (P < 0.01), (2.20 ± 0.30/3.30 ± 0.40) mm (P < 0.01) and (2.40 ± 0.40/3.40 ± 0.40) mm (P < 0.05) respectively. The incidence of shallow anterior chamber in group D was 3/15, and no flat chamber was seen in group B and C. CONCLUSION: Phacoemulsification reduce IOP and increase chamber depth in senile cataract or filtration-induced cataract of SPACG patients. It is as effective as trabeculectomy in the selective SPACG, and it offers a more safe and easier way to control IOP. LA: Chinese
