Clinical investigation on application of tissue plasminogen activator in glaucoma filtering surgery

OBJECTIVE: To study the effect of tissue plasminogen activator (t-PA) on glaucoma filtering surgery. METHODS: A randomized control clinical trial was curried out in 76 patients (86 eyes) with refractory glaucoma received trabeculectomy with and without t-PA. 4 μg of t-PA was intraoperative application and postoperative subconjunctival injections respectively in 44 t-PA-treated eyes, and control eyes (42) received no t-PA. The follow-up period was 18 to 48 months, and the data were analyzed by using the life-table method of Kaplan-Meier. RESULTS: The cumulative success probability in t-PA group was 84.2% and that in control group was 52.9% with significant difference (P < 0.05). The cumulative percentage of functional bleb in t-PA group was 84.5% and that in control group was 50.9% (P < 0.05). Conjunctival wound leakage rate were 2.2% and 2.3%, and hyphema of anterior chamber was 20.4% and 26.8% (P > 0.05) in t-PA group and control group, respectively. There was an insignificant change in corneal endothelial density following the use of t-PA (P > 0.05). CONCLUSIONS: t-PA is a safe and effective agent in glaucoma filtering surgery through considerable increases in success probability and by maintaining the postoperative IOP at relatively low level for 3 years.
