Dramatic decrease in intraocular pressure following topical administration of cycloplegics in an aphakic child

Aphakic children are at increased risk of developing glaucoma. In the very young patients, sedation or examination under anesthesia may be necessary to measure the intraocular pressure. Often the child is dilated prior to anesthesia so cycloplegic retinoscopy and assessment of the fundus also can be accomplished while the child is sedated. We report a case of dramatic decrease of intraocular pressure in an aphakic child after the topical administration of cycloplegic drops. If a child is dilated prior to measurement of intraocular pressure, the intraocular pressure may be substantially lower compared with when the patient is not dilated. We suggest that even with examinations under anesthesia, the intraocular pressure be measured prior to dilation in aphakic children.
