Increase of intraocular pressure after topical administration of prostaglandinanalogs.

this is a report of a 29 years old woman with retinitis pigmentosa ad juvenile glaucoma classified as Rieger Syndrome. The initial IOP at the time of diagnosis was 50 mmHg. Both eyes had an Elliot operation followed by cryocoagulation. The left eye had pressures between 0 and 41 mmHg on maximum tolerated medical therapy. When the patient had regular pressures of 30 mmHg the authors tried unoprostone. The IOP immediately increased to 56 mmHg. After withdrawal of treatment the IOP returned to 15 mmHg. Later on latanoprost became available. Also with latanoprost the IOP increased immediately to 55 mmHg. More cyclodestructive procedures followed. They resulted in a decreased visual acuity and a pressure of 0 mmHg. In order to raise the pressure the patient received drops with unoprostone after which the pressure increased to 55 mmHg again with improvement of the visual acuity. Of course the authors offer no explanation but they feel that the altered scleral outflow may have been involved in this patient or the effect of cyclodestructive procedures.
