Surgical management of coexisting glaucoma and cataract

Visual recovery is slower, intraocular pressure is higher, bleb survival is lower, and the need for antiglaucoma medications is greater when cataract surgery is combined with trabeculectomy. Sequential (staged) surgery for eyes with coexisting open-angle glaucoma and cataract is a safe and useful option for the surgeon. Cataract extraction is performed using a temporal clear corneal phacoemulsification approach, either prior to or after an antifibrotic trabeculectomy. Staging maximizes the benefits of each surgical component, while minimizing complications.
