This is a report of a 74 year old man with primary open angle glaucoma who had a trabeculectomy and a repeat trabeculectomy with 5-fluorouracil in his right eye. Not withstanding the operations the pressures remained 26 mmHg. Latanoprost reduced the pressure from 26 mmHg to 12 mmHg. He then had a phacoemulsification in this eye and the pressure went to zero with a large choroidal detachment. After stopping latanoprost the pressure increased to 12 mmHg without treatment. The authors discuss some of the literature on hypotony. They comment that there was no uveitis in this particular case. They feel that latanoprost may have had a contributory role in this hypotony. However they did not perform a rechallenge. They end with the usual line that clinicians should be aware of this potential latanoprost complication. The abstract however did not proof a cause and effect relationship between latanoprost and the complication.
