Bilateral Angle Closure Glaucoma and Ciliary Body Swelling from Topiramate

Topiramate is used for treatment of partial - onset seizures. This is a report of a 43 years old who went to a doctor with blurred distant vision and mild frontal headache. She was found to have a 5 diopter myopia, narrow angles, shallow anterior chamber and pressures of 30 mmHg. After timolol treatment the pressure was reduced. The central anterior chamber depth was 1.4 mm and the distance from the lens to the macula 15.6 mm. High frequency B-scan showed swelling of the ciliary body and forward displacement of the lens. After stopping topiramate and later also of timolol the visual acuity went back to normal without correction and intra-ocular pressure was 10 mmHg. Also angles were open and anterior chamber depth returned to 2.5 mm. The point made by these authors is that they have documented ciliary body swelling and associated forward migration of the crystalline lens. These effects of topiramate have been reported before.
