The authors reviewed a consecutive series of 299 eyes of glaucoma that underwent trabeculectomy with mitomycin C over 26 months. Trabeculectomy alone was performed in 160 eyes, and simultaneous cataract surgery was performed in 139 eyes. Five eyes of five patients (1.7%) developed bleb-related endophthalmitis. The interval between surgery and endophthalmitis ranged from five to 19 months, average 13.8 months. Prior to endophthalmitis, four eyes had shown aqueous leakage from the bleb and three eyes had shown defective conjunctiva over the bleb. The findings show that bleb leak and conjunctival defect are major risk factors of late bleb-related endophthalmitis following trabeculectomy with mitomycin C. LA: Japanese
