In this letter to the editor, the authors comment on a paper by Hayashi et al. on changes in anterior chamber angle width and depth after intraocular lens implantation in eyes with glaucoma. The corresponding authors particularly consider the extent of peripheral anterior synechiae. They feel that the angle only really opens after synechio-goniolysis in these cases. They report a study in which they have performed ultrasound biomicroscopic examination of the angle in cases with 360° peripheral anterior synechiae after chronic angle closure. They performed the ultrasound before and after phacoemulsification. They report that the trabecular meshwork was exposed to the aqueous only in the inferior 180° of the angle. Evidently, in the superior 180°, no synechiolysis was performed.
