The effect of trabeculectomy on corneal astigmatism

Recently surgery for glaucoma is being performed at early stages of the disease when the vision is still fairly good. Many patients report subjective changes in vision following trabeculectomy. Surgically induced astigmatism is becoming an important issue because it delays visual rehabilitation and exerts a negative effect on the final visual outcome. The study was conducted on 100 patients, 86 males and 14 females, whose age ranged from 35- 75 years. Two types of astigmatic changes were found following trabeculectomy. 87 % of patients showed vertical steepening and 'with the rule' astigmatism, while 13 % showed vertical flattening and 'against the rule' astigmatism. The surgically induced astigmatism was more in the early post operative period and decreased with time. The astigmatism lasted till the end of the study which was a period of three months. The impact of the procedure on the visual prognosis of the glaucoma patient must therefore be carefully evaluated.
