Evaluation of surgical effectiveness for neovascular glaucoma

OBJECTIVE: Neovascular glaucoma has been reported to occur in many ocular diseases. Most of these diseases have retina ischemia as a common inciting factor. This study was to observe the clinical outcomes of different surgical managements for neovascular glaucoma. METHODS: Forty-one eyes of 40 cases with neovascular glaucoma received different surgical managements were analyzed retrospectively. All cases were divided into three groups. Group A: Trabeculectomy with mitomycin C (MMC) was performed in 13 eyes of 12 patients. Group B: Cryotherapy of peripheral retina and ciliary was performed in 13 eyes of 13 patients. Group C: Combination of trabeculectomy and cryotherapy of peripheral retina and ciliary was performed in 15 eyes of 15 patients. RESULTS: The intraocular pressure (IOP) were controlled to (20.08 ± 7.11) mmHg in nine patients in group A and (21.50 ± 7.28) mmHg in eight eyes in group B with a surgery success rate of 69.1% and 61.5%, respectively. The IOP was controlled to (19.43 ± 7.98) mmHg in 12 cases in group C and the surgery success rate was 80%. The operative complications included hyphema, pain and atrophy of eyeball. CONCLUSION: Various surgery techniques for neovascular glaucoma should be chosen depending on the different conditions as follows: Trabeculectomy with MMC is suitable for the patients with visual acuity, clear refractive media and a little neovascularization on the surface of iris. Fundus fluorescence angiography-assisted photocoagulation is more effective in the eyes of two weeks after operation. Cryotherapy of peripheral retina and ciliary is better for the eyes without visual function, and combination of trabeculectomy and cryotherapy of peripheral retina and ciliary is optimum in the patients with visual acuity but refractive media opacity.
