This paper deals with specific problems of headaches in seniors. The occurrence of certain primary headaches (migraines, tension headaches) decreases with increasing age, the occurrence of secondary headaches (organic or metabolic diseases) increases, certain types of headaches occur only in the elderly (giant cell arteritis, hypnic headaches). Differential diagnosis of migrenous aura and transient ischemic attack is described and specific problems of acute and prophylactic treatment in migraines and other primary headaches in the elderly age are mentioned. Secondary headaches are described, which have increased significance in the elderly as expansive procesors (tumors, chronic subdural hematomas), cerebrovascular diseases, giant cell arteritis, metabolic headaches and headaches associated with systemic diseases, medication-related headaches, trigeminal neuralgia, postherpetic neuralgia, cervicogenic headache and glaucoma.
