Latest guidelines of ESC/ESH recommend to decrease blood pressure in patients with hypertension and diabetes mellitus below 130/80 mm Hg, or even lower during the night. The problem is with hypotension during the night in this patients, because it may increase the risk of development of glaucoma, particularly in the case of regulation dysfunction of optic circulation. There is a need for further randomised study, to explain the role of hypotension during the night in progression of glaucoma in patients with diabetes mellitus. These study ought to establish safe diastolic blood pressure value after hypertension treatment at night in patients with hypertension, diabetes mellitus and glaucoma.
