AIM: To investigate the clinical value of paracentesis of anterior chamber in glaucoma therapy. METHODS: Before or during glaucoma surgery, 210 patients (239 eyes) were treated by paracentesis of anterior chamber to reduce intraocular pressure (IOP). Of them, 22 eyes with acute angle-closure glaucoma presented acute paroxysm and increased IOP which could not be controlled by drugs. After the treatment of paracentesis of anterior chamber, IOP significantly decreased; in the other 217 eyes, aqueous humor was drawn out by paracentesis of anterior chamber during trabeculectomy. At the same time, balanced saline was injected into anterior chamber to induce the formation of anterior chamber and filtering bleb, adjust the IOP and prevent complication. RESULTS: The symptom of acute IOP became better or disappeared rapidly after drawing out aqueous humor via paracentesis of anterior chamber. In addition, the IOP decreased and the vision recovered gradually. After the treatment of trabeculectomy, except that 5 cases who had shallow anterior chamber in I degree, all the other patients did not have related complication. CONCLUSION: Paracentesis of anterior chamber in glaucoma therapy is an effective approach to reduce IOP and prevent the complication during and after the surgery of trabeculectomy.
