OBJECTIVE: To observe the effect of length of time of preoperative topical administration of 1% pilocarpine on the conjunctival tissues. METHODS: Conjunctival biopsy of 88 patients were examined by light and electron microscope, cellular type and numbers were analyzed quantitatively. The patients were divided into three groups according to the period of topical medication: within one year (group 1, n = 40), between 1 ≤ 3 years (group 2, n = 23), more than three years (group 3, n = 25). RESULTS: In comparison with normal conjunctival tissues, there were no appreciable alterations in group 1, lymphocyte activation and slight increase of inflammatory cells and fibroblasts in the subconjunctival tissue were found in group 2, the inflammatory cells and fibroblasts were significantly increased in group 3, while some cases in group 3 showed conjunctival epithelial hyperplasia, fibrosis and subclinical inflammation. CONCLUSIONS: Long-term administration of 1% pilocarpine can induce a inflammatory reaction and collagenous fibers hyperplasia of the conjunctival tissues, particularly in the patients with more than three years of topical medication.
