Purpose: The Icare ONE (Icare Finland Oy) handheld rebound tonometer, designed for adult home self-tonometry is a less expensive, computer-linkable version of the original Icare device. No studies have compared Icare ONE to conventional tonometry. The purpose of this study is: 1) to compare Icare ONE tonometry by clinic examiner and parent/guardian to Goldmann applanation tonometry(GAT) in children with known or suspected glaucoma; 2) to use Icare ONE for home tonometry in selected children. Methods: Ongoing prospective study of children from the pediatric glaucoma clinic. Intraocular pressure(IOP) is measured using Icare ONE(registered trademark) by both a clinic examiner and the child's parent/guardian, then by GAT (masked physician). Parent/guardian then completes ease-of-use survey. Mean1+-SD, p-value(two-tailed) presented unless otherwise stated. Results: Included thus far are 20 eyes (20 children), mean age 10.8 years (5-16). For Icare ONE mean IOP difference [clinic examiner vs parent] was 1.6 (plus or minus) 1.9 mm Hg (p=0.4). Mean IOP difference [Icare ONE clinic examiner vs GAT] was 2.6 (plus or minus) 2.4 mm Hg (p = 0.07). Icare ONE IOP was within 3 mm Hg of GAT in 70%(n514 eyes); in eyes with .> mm Hg difference, Icare ONE was higher than GAT in 83%. All parents accomplished Icare ONE tonometry; 95% reported it was nulleasy to learn to usenull and better tolerated than GAT. Variables affecting IOP agreement (nystagmus, cooperation, device settings) will be assessed. Recruitment for home tonometry continues. Discussion: Icare ONE is well-tolerated and useable by parents, measuring IOP similar to GAT in 70% of cases. This is comparable to standard Icare in children. Conclusions: Icare ONE appears accurate compared to GAT, and holds promise for clinic and home tonometry in children.
