Management of cataracts in patients with glaucoma

Introduction: Cataracts and glaucoma are 2 of the most common ocular diseases and they frequently coexist as the prevalence of each increases with age.(1,2) The treatment of a patient's glaucoma may also increase the severity of his/her cataract. For instance, patients with glaucoma randomized to receive betaxolol and argon laser trabeculoplasty in the Early Manifested Glaucoma Treatment Trial had a significantly higher rapid development of nuclear opacities.(3) In the Collaborated Intervention Glaucoma Treatment Study, initial surgical treatment resulted in the development of more cataracts that required removal than initial medical treatment. The crude proportion of cataract extraction in the surgical group (17.3%) was almost 3 times greater than the proportion in the medicine group (6.2%).(4) As a result of the interaction of these 2 diseases, different surgical treatment options are available and ophthalmologists often find it difficult to determine the best recommendation for a patient.
