We evaluated serum homocysteinemia and resistance to activated protein C (APC) in a patient with normal tension glaucoma, migraine, and Raynaud's phenomenon. The patient's clotting time ratio was 1.95 (normal range is greater than 2.2). Polymerase chain reaction showed the patient was heterozygous for APC resistance. A serum homocysteine level was elevated. APC resistance and homocysteinemia may play an important role in diminished vascular perfusion to the optic nerve head.
