In this letter to the editor the authors describe their immediate experience with argon laser iridoplasty in a study concerning 8 patients which are in addition to an earlier publication concerning 10 patients. All 8 patients were of Chinese ethnic origin with dark brown irides. The duration of the attack prior to presentation ranged from 7 to 48 hours. They all had corneal edema. Nevertheless the treatment was possible. IOPs ranged from 45 to 73 mmHg. In almost all the patients the IOP came down satisfactorily. Only one patient still had a pressure over 50 mmHg. This patient needed a cataract extraction. The authors conclude that immediate argon laser iridoplasty is a save and effective means of bringing down IOP in patients with acute attacks of primary angle closure glaucoma. Topical anti-glaucomatous medications may have a synergistic effect. Their exact role still has to be evaluated.
