These authors describe a 48 years old woman who was first seen for an irregular pupil of the left eye. She had a good visual acuity in both eyes. Two years later the patient had irregular pupils in both eyes. There was inferior displacement in the right eye and superior and inferior displacement in the left eye. Islands of fine beaten silver appearance of the corneal endothelium were observed in both eyes. Mild iris stroma atrophy was also seen. Broad based peripheral anterior synechiae were seen in both eyes. The observation of bilateral ICE syndrome is rare. Subclinical involvement of the corneal endothelium in the contralateral eye is commonly observed. Although rare, such cases should not be surprising, a viral etiology has been postulated.
