Malignant glaucoma occurred 2 days after combined operations of lens extraction, intraocular lens implantation and trabeculectomy to treat primary angle colsure glaucoma in a 50-year-old female. This condition was treated by Nd: YAG laser capsulotomy and vitrectomy. Capsulotomy holes closed about 6 days after the treatment. In addition malignant glaucoma recurred about 6 days later. In cases of malignant glaucoma, abnormal aqueous flow routes most likely persist even after effective medical and laser treatments. Re-bockage of aqueous flow routes among anterior, posterior chambers and vitreous may easily induce recurrence of malignant glaucoma. Conforming that the capsulotomy holes are open should be important during follow-up examinations. In this case the eye had a shorter axial length and smaller cornea. We should be watchful for the possible onset of malignant glaucoma in atypical cases of angle-closure glaucoma in eyes with morphological abnormalities such as short axial length or plateau iris syndrome.LA: Japanese
