We observed 2 cases with showed increased iris pigmentation following topical instillation of isopropyl unoprostone. One was a 56-year-old male with bilateral open-angle glaucoma. Iris pigmentation in both eyes was detected 36 months after start of instillation. After discontinuation of instillation in the left eye following successful trabeculectomy, there was no further increase in iris pigmentation. The second case, a 64-year-old male with ocular hypertension in his left eye, iris pigmentation was detected 12 months after start of instillation. No further changes in the iris occurred after switching to instillation of 0.5% timolol.
