Limitations of the Glaucoma Hemifield Test in identifying early glaucomatous field loss

In early glaucoma, the Glaucoma Hemifield Test (GHT) interpreted 27% of 1676 fields as 'within normal limits' despite the presence of real scotomas on numeric pattern deviation displays (p < 0.0001). In early glaucoma, visual field analysis based on the GHT alone is at significant risk for misinterpretation, underdiagnosis, and under-treatment. In glaucoma suspects, the defect depth or pattern deviation displays should always be examined before any diagnostic or therapeutic decision.
