A case of angle-closure glaucoma with largely fluctuating intraocular pressure

A 52-year-old woman was diagnosed with angle-closure glaucoma due to an intumescent clear lens in her right eye. The elevated intraocular pressure (IOP) persisted after laser iridotomy and laser gonioplasty. Despite additional core, vitrectomy and goniosynechialysis resulted, and IOP fluctuated greatly. The affected eye underwent lens extraction and intraocular lens (IOL) implantation for cataract nine years after the initial diagnosis. An in-the-bag fixated IOL was positioned anteriorly, resulting in a shallow anterior chamber, formation of peripheral anterior synechia (PAS), and marked IOP fluctuation.
