This is a report of a 52 year old healthy man with no known vascular risk factors. He had erectile dysfunction after transurethralresection for prostate cancer. His first dose of 50 mg of Viagra one evening and within one hour experienced sweatng and severe generalized headache. He saw blue "lightning bolts" and reported blurry vision in both eyes which lasted for 30 minutes. The vision in the left eye remained blurred inferiorly. He repeated the Viagra intake the next night with recurrence of the same symptoms. The blurry vision in the left eye did not change over time. The authors who saw the patient reported ischemic optic neuropathy particularly in the upper half, corresponding with a visual field defect in the lower half.
