Experience with early internal obstruction of the filtration site following filtration surgery.

These authors discuss their own experience with internal obstruction of the filtration site after trabeculectomy. They start by presenting 7 cases that they personaly treated. All seven patients presented with a loss of bleb and / or elevation of intraocular pressure 3 to 28 days following uncomplicated filtration surgery. In five of the seven cases laser treatment was successful in eliminating the obstruction at the filtration site. The authors conclude that they were unable to predict which patients are at increased risk of developing obstruction of their filtration site during the first month following surgery. If the internal obstruction of the filtration site is detected before the bleb has scared down, laser treatment can be used to relieve the obstruction, restore aqueous outflow to the bleb, and maintain reduction in intraocular pressure. The key is to consider the diagnosis, performed gonioscopy to confirm it, and then treat with the laser.
