Long-term results of diode versus Nd:YAG contact transscleral laser cyclophotocoagulation for refractory glaucoma

Ninety-eight eyes with advanced refractory glaucoma underwent transscleral laser cyclophotocoagulation (TLCP), of which 48 eyes were treated by Nd:YAG laser and 50 by diode laser. The laser probe was placed 1.5 mm behind the limbus, using 4-6 W of power for 0.5 seconds with 60 laser burns and 2 W of power for 1.5-2 seconds with 40 laser bums, in each eye in the Nd:YAG group and the diode group, respectively. The mean number of treatments was 1.50±0.62 in the Nd:YAG group and 1.64±0.69 in the diode group (p>0.05). Mean follow-up after the first TLCP was 16.2±8.5 months. Compared to the baseline pressure, at the last follow-up examination, mean intraocular pressure (IOP) had decreased by 49% and the mean number of medications had been reduced by 61% in the Nd:YAG group, and the mean IOP had decreased by 45% and the mean number of medications had been reduced by 48% in the diode group, respectively. Seventy-seven patients in the Nd:YAG group and 82% of patients in the diode group were relieved of ocular pain (p>0.05). At the last follow-up visit, 50% of eyes in the Nd:YAG group and 54% in the diode group (p>0.05) had a final IOP of between 3.96 and 28.01 mmHg. The mean IOP values were similar in the two groups within the first 30 months, but this success decreased with increased duration of follow-up, from 73 to 43% and from 76 to 46% in the Nd:YAG and diode groups, respectively. Four eyes (8%) in the Nd:YAG group and two eyes (4%) in the diode group developed hypotony (p>0.05). Eight eyes (19%) in the Nd:YAG group and four eyes (9%) in the diode group finally lost light perception (p>0.05). These results suggest that late failure of TLCP is common and that both Nd:YAG and diode TLCP have similar outcomes.
