Congruous homonymous hemianopia superimposing glaucomatous scotoma

The authors describe a case of congruous homonymous hemianopia with open-angle glaucoma. A 71-year-old woman presented with bilateral blurred vision. On examination, corrected visual acuity was 0.4 in the right eye and 0.6 in the left eye. Intraocular pressure was 22 mmHg in the both eyes. Funduscopic examinations revealed vertical elongation of the physiological cup and thinning of the inferotemporal rim in the right optic disc. Visual field examinations revealed congruous right homonymous hemianopia, however, the upper central visual field defect was over the vertical meridian in the right eye. CT scans revealed a low-density area in the left occipital cortex. These clinical findings suggest that this unique visual field defects could be produced by a cerebral infarction in the left visual cortex and glaucomatous scotoma reflecting the central arcuate nerve fiber-layer defect in the right eye.
