Acute angle closure glaucoma presenting in a young patient after administration of paroxytone

A 40-year-old man presented at the casualty department with blurred vision and discomfort in the right eye. Two weeks earlier he had started oral paroxytone. On examination reduced vision was found, and right mild inferior edema. IOP was 56 mmHg in the right eye. Gonioscopy revealed complete angle closure. The unaffected eye had a very narrow drainage angle. Paroxytone is a drug licensed for the treatment of depression, etc, which is reported to have few if any anti-cholinergic side-effects. To date, three elderly patients have been reported to have acute angle closure glaucoma secondary to this drug.
