Pigment dispersion syndrome and butterfly-shaped pattern dystrophy of the retinal pigment epithelium

A 48-year-old woman presented with good visual acuity in her right eye and 20/25 in her left eye. She had a typical Krukenberg Spindle and pigmentation of the trabecular meshwork. There was also a deposit of pigment on the posterior surface of the lens. The macula showed a butterfly-shaped yellowish reflex in the deeper layers of the retina. This case showed the characteristic features of pigmentary dispersion syndrome with normal intraocular pressure, visual fields and optic disc, together with butterfly-shaped dystrophy. This association may be regarded as a coincidence, but it may also be regarded as a different expression of the same pigment epithelial dystrophy.
