A 54-year-old woman underwent filtering surgery for a seventh time in her left eye for refractory open-angle glaucoma. The bulbar conjunctiva had become very thin suggesting a high risk of bleb leak after conventional filtering surgery. Therefore, a free conjunctival piece was excised from her right blind eye to provide a support to the filtering bleb. Mitomycin C was used adjunct to the seventh filtering surgery. A well-developed filtering bleb resulted with no aqueous leakage. The intraocular pressure has been well controlled with topical timolol. This case illustrates the usefulness of free autologous conjunctival tissue transplant in treating thin conjunctival bleb over the filtering bleb. LA: Japanese
