Surgical results of proliferative diabetic retinopathy with iris-angle rubeosis

PURPOSE: To evaluate the surgical results of primary vitrectomy with glaucoma surgery as needed for proliferative diabetic retinopathy (PDR) with iris-angle rubeosis on vision and intraocular pressure (IOP). METHODS: The 39 eyes in this study were divided into two groups: 24 eyes without high IOP preoperatively (group A: non-glaucoma), and 15 eyes with high IOP preoperatively (group B: glaucoma). The authors compared the prognosis for vision and IOP in the two groups after primary vitrectomy for PDR with iris-angle rubeosis, with (group B) or without (group A) glaucoma surgery. RESULTS: After surgery, IOP was well-controlled below 21 mmHg in 21 (88%) of the eyes in group A and in 12 (80%) of the eyes in group B with or without topical treatment. Vision improved after surgery in the two groups, and the difference between the two groups regarding visual outcome was not significant. CONCLUSIONS: Even if IOP is not controlled preoperatively, vitrectomy with glaucoma surgery can result in relatively good visual prognosis and control of IOP in eyes of patients with PDR with rubeosis.
