Trabeculectomy combined with clear lens extraction for primary angle-closure glaucoma

OBJECTIVE: To study the indications and efficacy of trabeculectomy combined with clear lens extraction for primary angle-closure glaucoma (PACG). METHODS: Twenty-six eyes with clear lens were treated with trabeculectomy combined with phacoemulsification and foldable intraocular lens implantation through one incision. The follow-up period was 12-18 months. RESULTS: The average intraocular pressure (IOP) was reduced from 29.82 ± 7.82 mmHg preoperatively to 13.94 ± 5.76 mmHg postoperatively. IOP was controlled at under 21 mmHg in 88.46% of operated eyes. The visual acuity of 50% of eyes was 0.5 or better. The preoperative axial length was 22.66 ± 0.96 mm, lens thickness 4.86 ± 0.22 mm, anterior chamber depth 2.35 ± 0.28 mm, and postoperative anterior chamber depth 4.10 ± 0.37 mm. The incidence of shallow anterior chamber was lower than in the control group, which received trabeculectomy only. CONCLUSIONS: The triple procedure provides good results in PACG with a thick lens, shallow anterior chamber, and short axial length. LA: Chinese
