Filtering bleb failure of open angle glaucoma

PURPOSE: To analyze the frequency and causes of trabeculectomy failure in primary and secondary open-angle glaucoma and the results of individual treatment in these cases. METHODS: A retrospective study of 69 trabeculectomy failures. Filtering bleb failure was recorded in 49 cases of primary open-angle glaucoma and in 21 cases of secondary open-angle glaucoma. Slit-lamp examination and gonioscopy clarified the cause of the failure. Treatment was established in an individual manner in order to achieve stable control of intraocular pressure (under 21 mmHg) RESULTS: The cause of early failure (15 cases) was obstruction of the internal ostium of the trabeculectomy in ten cases, episcleral and conjunctival fibrosis in four cases, and encapsulated bleb in one case. Episcleral and conjunctival fibrosis in 46 cases and encapsulated bleb in six cases caused late failure. The most effective option after early failure of trabeculectomy was internal de-obstruction. In cases of late failure, additional medical therapy and a second filtering surgery, combined with antifibrotic agents, were the most effective and commonly used techniques. CONCLUSIONS: Immediate recognition of the failure, ascertaining the etiology and the appropriate treatment gives a high rate of therapeutic success.LA: Romanian
