Editorial Comment: Surgical management of the glaucomas

this is an editorial comment on the use of Molteno implants by Molteno and Dempster. These authors were interested in studying tissue response to aqueous humor in a controlled filtration bleb and second in the implants ability to reduce intraocular pressure. They would also like to compare the use of the implant with the results of trabeculectomy. It seems that in the absence of metabolic agents the intraocular pressure control is quite similar in the group of complicated glaucomas receiving a Molteno implant and in the group of non-complicated glaucomas having a trabeculectomy. There is special emphasis on the control of bleb inflammation. Molteno recommends the use of an oral anti-inflammatory drug regime also for trabeculectomy. Implants may have the major advantage of being less prone to some of the devastating complications of antimetabolites. This particular editor seems to be rather adverse to the use of mitomycine-C. however he also points out that the chance of bleb leakage may be much lower in African than in Caucasian patients.
