Modern therapeutic approach to congenital abnormalities of the anterior segment in the infant eye

Modern functional and anatomical tests allow a reproducible approach to congenital abnormalities of the anterior segment in pediatric patients. Bidimensional ultrasonography allows accurate IOL determination to be performed, based on precise localization of the macula. In congenital glaucoma, it allows optic disc cupping to be evaluated when retinal biomicroscopy is not possible. Color Doppler ultrasonography enables the blood flow of intraocular vessels to be assessed (in case of persistence of primitive vitreous) or of retrobulbar vessels (in eyes with congenital glaucoma). Miniaturization has allowed a great number of devices to be adapted, such as the automatic refractometer, tonometer, retinograph and angiograph, for use in patients in the prone position. These techniques allow the anatomical and functional status of eyes undergoing surgery for congenital cataract to be assessed, which is based on anterior and posterior capsulorhexis followed by central vitrectomy, for congenital glaucoma, using modern goniotomy or external trabeculotomy, or for the anterior persistence of primitive vitreous treated with membranectomy.LA: French
