PURPOSE: To evaluate the results of trabeculectomy with a fornix-based conjunctival flap in eyes with cicatrization of the conjunctiva after multiple intraocular procedures. METHODS: There were six patients (six eyes) with open-angle glaucoma and two (two eyes) with neovascular glaucoma who had extensively cicatricial conjunctiva after past operations (average, 2.6 operations per eye). The patients' average age was 64.1 (range, 48-77) years, and the average follow-up period 16.3 (range, 8-30) months. Past operations included vitreous surgery (five eyes), encircling scleral buckling for retinal detachment (one eye), filtering operations (two eyes), and corneoscleral incision cataract surgery (seven eyes). In all cases, a fornix-based conjunctival flap procedure was performed in the upper portion (approximately the upper two quadrants) and Tenon's tissue was removed to the level of the superior rectus muscle. In addition, 0.02% mitomycin C was used in each case. RESULTS: In all cases intraocular pressure (IOP) decreased significantly and a good cystic filtering bleb formed. Postoperative IOP did not exceed 21 mmHg in any case, and in seven cases an acceptable IOP was maintained without medication. There was no serious perioperative or postoperative complication. CONCLUSIONS: This procedure involving wide incision of the conjunctiva may promote formation of a good filtering bleb and be an effective way to manage cases of cicatricial conjunctiva. LA: Japanese
