Clinical study of refractory glaucoma treated with Molteno implantation

OBJECTIVE: To evaluate the effect of treating refractory glaucoma with a Molteno implantation. METHODS: Molteno implantation was used to treat refractory glaucoma in 24 cases (24 eyes). RESULTS: The IOPs of 18 cases returned to normal levels within a month after the operation. The curative rate reached 75.0%. There was a significant difference between the preoperative and postoperative IOPs (p < 0.01). The length of drug treatment for patients was significantly reduced after the operation. The effect of the operation was affected by the mode of implantation. One-phase or two-phase implantation combined with restriction of the aqueous humor from flowing out, obtained the best curative rates (75-83%). In these patients, 16 eyes were surveyed more than five months after the operation, and the IOPs in 11 eyes had returned to normal. The curative rate was 68.75%. The main complications were the low IOP postoperatively and the flat anterior chamber, which could be avoided by two-phase implantation or by one-phase implantation combined with restriction of the aqueous humor.
