Evidence-based treatment for operative fashion of acute angle-closure glaucoma

OBJECTIVE: Acute angle-closure glaucoma is one of the common eye diseases inducing blindness. How to improve the visual function and reduce blindness rate of acute angle-closure glaucoma is constantly a hotspot for scholar to research. The aim of present study was to analyze and explain how to treat a patients with angle-closure glaucoma by surgery based on best clinical evidence. METHODS: Cochrane library, Medline, EMbase, CBM disc, CJFD and VIP were searched to find the best clinical evidence and evaluate the quality of evidence. RESULTS: Three randomized controlled trials (RCTs) and 2 controlled clinical trials were found to be eligible, so these data were evaluated. The searched outcome showed that primary phacoemulsification and intraocular lens implantation were more safe and effective in reducing intraocular pressure and improving visual acuity than peripheral iridectomy and Nd:YAG laser iridotomy in patients with acute angle-closure glaucoma with the lower secondary operative rate. Based on the review of above evidences, primary phacoemulsification and intraocular lens implantation was applied in the patients with angle-closure glaucoma, and a visual acuity of 0.5 and intraocular pressure of 17.3 ± 2.8 mmHg were retained during the 3-year following-up. CONCLUSION: Primary phacoemulsification and intraocular lens implantation will be better choice for acute angle-closure glaucoma. LA: Chinese
