Cause of secondary glaucoma and its management after silicone oil removal combined with perfluoropropane gas plombage

AIM: To discuss the cause of secondary glaucoma and its management after silicone oil removal combined with perfluoropropane gas plombage. METHODS: Patients from January 2004 to October 2008 treated by with complex vitreoretinal diseases vitrectomy and silicone oil inject were reviewed. Silicone oil in the eyes of patients under fundus examination revealed fresh bleeding lesions, the proliferation of film, macular frontal membrane associated with macular edema, new holes. Secondary glaucoma occurred after silicone oil removal combined with perfluoropropane (C(3)F(8)) gas filling. RESULTS: Forty-six eyes occurred secondary glaucoma 1 week to 3 months postoperatively: 5 eyes occurred cataract (5/46); 2 eyes anterior chamber angle adhesion (2/46); 1 eye atrophia bulbi (1/46); 3 eyes atrophia nervi optici (3/46); 8 eyes long-time elevated intraocular pressure (IOP) (8/46); 4 eyes corneal endothelium edema (4/46). The common causes were excessive injection of C(3)F(8) gas, high concentration, wide anterior synechiae of iris and disappearance of anterior chamber caused by overfilling of no crystal vitreous cavity and iris bombe; incorrect postoperative position; eye inflammation; remaining drops of silicone oil emulsion, such as elevated IOP affected by the combined effect of oil and gas. Among the total, 34 eyes were released some gas intraoperatively and given anti-inflammatory corticosteroids, IOP lowering drugs. And IOP returned to normal after treatment; gas releasing and anterior chamber plasty were performed in 2 eyes, the IOP of which returned to normal; remnant of silicone oil emulsion droplet occurred in 4 eyes, IOP was controlled well by the the joint use of SLT laser treatment and IOP lowering drugs. Atrophy of eyeball occurred in 1 eye after gas releasing. CONCLUSION: Silicone oil removal combined with C(3)F(8) gas can cause secondary glaucoma. But after effective treatment, satisfactory results can be achieved. LA: Chinese
