Introduction: When glaucoma shunt devices are used in pediatric eyes, the tube may become progressively short within the anterior chamber as the child grows. Tube extenders have been developed to correct this problem. Methods: We report 2 cases with adverse outcomes after use of tube extenders. Results: We report 2 cases in which progressive fibrotic proliferation caused the tube extender to detach from the primary tube over time. The children presented with increased intraocular pressure, absence of a bleb over the plate, and a cystic appearance at the proximal end of the tube extender. The detachment of the tube extender was documented by ultrasound biomicroscopy and confirmed intraoperatively. Conclusions: Although tube extenders offer a solution to the problem of a short glaucoma shunt tube within the anterior segment, progressive fibrosis can result in detachment of the tube and failure of aqueous drainage. This complication has not previously been reported.
