Outcome of nonpenetrating lamellar trabeculectomy in Department of Ophthalmology, Tottori University

Nonpenetrating lamellar trabeculectomy (NPLT) due to its high degree of success in decreasing intraocular pressure (IOP) without serious complications, has been noted as a useful surgical procedure for patients with primary open-angle glaucoma (POAG). We analyzed the results of recently performed NPLT at Department of Ophthalmology, Tottori University. The subject patients were 29 patients (41 eyes) with POAG aged 49 to 88 years, averaging 70.4 years at surgery. NPLT was undertaken in 17 cases (27 eyes), and combined with phacoemulsification aspiration and intraocular lens implantation in 12 cases (14 eyes). Mitomycin C was used after making the initial scleral flap. The operations were performed from October, 1996 to January, 1998, the length of follow-up ranging from 0.5 to 14 months, averaging 3.6 months. Postoperative IOP level decreased significantly from the preoperative level of 23.5+-5.8 mmHg (standard deviation) to 14.8+-5.2 mmHg at 1 month after surgery, and to 15.9+-3.6 mmHg at the end of follow-up (p<0.0001 in each term), with almost no medication in all cases. Postoperatively, looseness of conjunctival sutures and leakage of aqueous humor from the conjunctival wound were observed in 7 eyes (6 cases) and 5 eyes (4 cases), respectively, but such minor complications disappeared naturally, excepting 1 case of conjunctival resuturing. No serious complications, such as anterior chamber disappearance, hypotensive maculopathy, hyphaema, transient IOP elevation, choroidal detachment and so forth, were detected in any case throughout the course. Based upon these findings, NPLT is regarded as an effective initial procedure in patients with POAG, although exposure and elimination of the inner wall of Schlemm's canal with the outer part of the trabecular meshwork require great skill in the surgical procedure.LA: Japanese
