Two cases of lens particle glaucoma occurring long after cataract surgery

The authors treated two patients with lens particle glaucoma that occurred many years after cataract surgery. In case 1, a 64-year-old female developed lens particle glaucoma in her left eye 11 years after cataract surgery (phacoemulsification and aspiration). Aspiration of residual lens cortex and irrigation of the anterior chamber was performed, and the intraocular pressure (IOP) has remained in the normal range. In case 2, a 53-year-old female developed lens particle glaucoma in her right eye 23 years after cataract surgery. Medical treatment was chosen at first because of posterior capsule rupture and aphakia, but natural absorption did not occur and IOP control was poor, so trabeculectomy was ultimately performed. The residual lens cortex seemed to have been covered with lens capsule and be separated from the aqueous humor, but the residual lens broke and lens particle glaucoma developed. Removal of a residual lens and irrigation of the anterior chamber is necessary early when medical antiglaucoma and anti-inflammatory therapy are not effective. LA: Japanese
