Surgical treatment for the complications of persistent hyperplastic primary vitreous

The authors performed surgery on five eyes of four patients with complications of persistent hyperplastic vitreous (PHPV). Patient 1 underwent pars plicata lensectomy and vitrectomy for secondary glaucoma. In patient 2, cataract extraction was performed successfully by lens aspiration and anterior vitrectomy by a limbal approach. Bilateral retinal detachments in patient 3 were treated with cryopexy and an encircling buckle. In patient 4, who had retrolental fibroplasia, lensectomy and vitrectomy via a limbal approach was performed but was not successful. Moderate or severe posterior retinal pathology was present in all of the five eyes. In such cases, lensectomy and/or vitrectomy by a limbal approach is recommended to avoid retinal insertion into the sclerotomy and retinal dialysis. To treat retinal detachment, a transscleral or transvitreal approach should be chosen, depending on the degree of vitreous opacity and proliferative changes. LA: Japanese
