Assessment of a clinic glaucoma screening program

PURPOSE: To review the results of a clinic screening program for glaucoma in the community. METHODS: A community screening for glaucoma was carried out at Kohjunkai OBP Clinic from January to December 1999. Each person's intraocular pressure (IOP) was measured using a noncontact tonometer, and each person underwent nonmydriatic fundus photography. If the IOP was higher than 22 mmHg or the fundus photograph showed glaucomatous disc cupping or retinal nerve fiber layer defect, the initial screening results were considered positive for glaucoma. RESULTS: A total of 5263 persons underwent the initial screening, and the results were positive in 10.1% (531/5,263). Of the 531 with initial positive findings, only 19.2% (102/531) returned for further evaluation. Of these 102 persons, the true positive glaucoma rate was 61.8% (63/102). Thus, glaucoma was detected in 1.2% (63/5,263) of all persons screened. Of these 63 persons, open-angle glaucoma was diagnosed in 19.0%, normal-tension glaucoma in 57.2%, primary angle-closure glaucoma in 1.6%, secondary or other types of glaucoma in 1.6%, and primary ocular hypertension in 20.6%. CONCLUSIONS: A major problem with this screening program was the very low proportion of patients with positive results who returned for further evaluation. Finding efficient methods of informing patients of their screening results is an important issue for future screenings.
