All too often glaucoma patients are diagnosed when much their visual func tion is lost. Screening seems to be mandatory. This review of the literature by Hirneiss et al. (1472) explains screening criteria according to Wilson, describes screening methods and the importance of high sensitivity and specificity and discusses two screening models from Finland (Vaahtoranta-Lehtonen) and Great Britain (Hernandez) with their extremely discrepant calculations of cost efficiency for screening. It is a good introduction into the pitfalls of screening.
