Management of late bleb leaks with a hinged partial thickness scleral
flap and conjunctional advancement effectively sealed the bleb leak.
(Jitem Mistry, Johannesburg)
The author used a lyoplant successfully after a late onset bleb
leak. (D.J.L. Coetzee, University of the Free State)
A home nebulizer using ipratropium bromide induced a unilateral
angle closure glaucoma. The mist had contact with the left eye causing
a pressure of 40 mmHg. The author suggests correct mask fitting, T-piece,
unvented masks, protective eye wear or closed eyes if none of the above
is available. (J. van Rensburg, Stellenbosch)
Encapsulation is an early complication in young patients leading
to inhibition of fluid drainage. It can be successfully treated with
surgical excision. The author reports of 20 complicated glaucoma cases
done in 2009. Thirty percent developed encysted blebs within six months
after surgery. The Ahmed implant was used which is known to develop
encysted blebs due to its rough surface. The author suggests early MMC
needling or surgical bleb excision improving the drainage and lowering
IOP. (N. Du Toit, Cape Town)
