Effect of selective laser trabeculoplasty on diurnal fluctuations of intraocular pressure in patients receiving secondary and repeat therapy

Background: To examine the effect of Selective Laser Trabeculoplasty (SLT) on the diurnal fluctuations of intraocular pressure (IOP) in patients receiving secondary or repeat therapy. Study: Retrospective chart review was performed on 109 eyes out of a consecutive case series of 2056 eyes treated with SLT. Eyes were selected for analysis which had follow-up IOP values of at least four years after secondary (adjunctive) or repeat therapy with SLT. There were 408 data points for eyes with secondary therapy and 500 data points for eyes with repeat therapy; with an average of 8.3 follow-up IOP values per eye. Results: Fluctuation in IOP vs. time was observed to have a logarithmic relationship. For eyes with SLT as secondary treatment, the morning and afternoon IOP values were significantly different (p<0.01). For eyes with SLT as repeat treatment, the morning and afternoon values were not significantly different (p>0.05). Conclusion: In this series, in eyes in which SLT was used as secondary treatment, the diurnal fluctuation of IOP appeared to be unaffected. In eyes in which SLT was used as repeat treatment, the diurnal fluctuation of IOP appeared to be eliminated.
