The effect of endotamponades on intraocular pressure in eyes operated by 23 gauge pars plana vitrectomy technique

Purpose: To asses the effect of endotamponades on intraocular pressure (IOP) after 23 gauge pars plana vitrectomy (23 gauge (G) PPV). Materials and Methods: We retrospectively reviewed the medical records of 40 eyes/40 consecutive subjects who underwent 23 G PPV in Beyoglu Eye Training and Research Hospital between February 2008 and January 2009. Silicon oil (1000 cst) was used in 17 eyes (Group 1), while gas endotamponade (C(3)F (8) and SF(6)) was used in 23 eyes (Group 2) at the completion of surgery. The change in IOP was assessed and compared in both groups at baseline and 1 day, 1 week, 1 month, 3 months and 6 months after the surgery. Results: In Group 1 the mean age was 48(plus or minus)15.8 years (8 female/9 male) and the mean follow-up was 6.4 (4-8) months. In Group 2 the mean age was 57(plus or minus)16.5 years (11 female/12 male) and the mean follow up was 7.4 (6-9) months. During the follow-up period in Group 1, 8 eyes (47%) had elevation in IOP controlled by medical therapy, 3 eyes (17%) had transient elevation in IOP and 1 eye (5.8%) had hypotonia while in Group 2 the rates were 2 eyes (8.6%), 6 eyes (26.0%) and 3 eyes (13%) respectively. The rate of elevated IOP controlled by glaucoma medication was significantly higher among cases in Group 1 (p=0.01). No significant difference was present in transient elevation in IOP (p=0.60) and hypotonia (p=1.00) between the groups. Conclusion: Elevation in IOP after 23 G PPV was more frequently seen in eyes with SiO while in eyes with gas endotamponade transient IOP elevation was more common. LA: Turkish
