Analysis of efficiency of open-angle glaucoma treatment by modified laser trabeculoplasty

PURPOSE: A clinical-functional estimation of open-angle glaucoma treatment by modified laser trabeculoplasty (LTP) carried out by ND: YAG laser (λ = 1064 nm), taking into account the anatomic arrangement of Schlemm's canal. Fifty eyes (40 patients) with open-angled glaucoma were operated. A 'vertical' arrangement of Schlemm's canal was marked in 32 eyes, a 'horizontal' disposition in 18 eyes. RESULTS: A decrease of intraocular tension (IOT) after use of modified LTP at 'vertical' Schlemm's canal, at that this state was more or less stable during a year after the operation (18-23 millimeter of mercury). In the other 18 cases a compensation of IOT wasn't observed. The analysis of research results has proved the efficiency of modified LTP by use ND: YAG laser in patients with a vertical arrangement of Schlemm's canal. It is explained by especial hydraulic properties of this position which is the worst for effective regulation of outflow and, hence, the carrying out LTP allows improving of intraocular liquid outflow.
